COVID-19 Visitor Screening and Check-in

Given the current circumstances regarding the COVID-19/coronavirus pandemic, anyone
entering the European Union Delegation premises is kindly required; to show proof of full
COVID-19 vaccination and fill in the following questionnaire. For the non-fully vaccinated, a
negative test taken within the preceding 72 hours is necessary. Failure to provide proof
means entry will be denied. Thank you for your understanding.

3. Are you fully vaccinated against COVID-19?

Fully vaccinated refers to people having received all shots of the COVID-19 vaccine (1 or 2 depending on the make) and who have waited 2 weeks after having received the final vaccine dose.

Partially vaccinated refers to people having received only one of the 2 shots of the COVID-19 vaccine (for makes requiring 2 doses) AND/OR who have not yet waited 2 weeks after receiving the final vaccine dose.

Unvaccinated persons: Have not initiated a COVID-19 vaccination series.

If Yes, please enclose a proof of vaccination to the present questionnaire.
If No, please enclose a proof of a negative test taken within the preceding 72 hours.

HEALTH SCREENING

If you answer yes to any of the questions below or present any symptom
consistent with COVID-19, please do not visit the EU Delegation.

4. Are you currently experiencing, or have you experienced in the past 14 days, any of the following symptoms? (Check box if yes)

5. Have you had a positive COVID-19 test in the past 10 days, or are you awaiting results of a COVID-19 test?

6. Within the past 14 days, has a public health or medical professional told you to self-monitor, self-isolate, or self-quarantine because of concerns about the COVID-19 infection?

If you have answered NO to all the health screening questions above and do not present any symptom consistent with COVID-19, you may visit the Delegation. The day of your visit, please confirm your answers have not changed and that you have not developed symptoms consistent with COVID-19.

During your visit, you are asked to:

  • Wash or sanitize your hands upon entering the EU Delegation;
  • Even if you have been vaccinated, please wear a mask covering nose and mouth;
  • Practice social distancing.

If visitors feel sick, develop symptoms consistent with COVID-19 or are diagnosed with COVID-19 in the next 2 to 14 days following their visit, they are kindly asked to inform:
